		<description>Sorry i have no data on Arizona but I know when I was looking around for a doctor I found this site that had some useful information.  I eventualy went witha doctor that had been doing the work for the longest and a very decent price.  The good with the doctor I went to is that he let me talk to some of his previous patients so that I could hear their perspective on how he was.  It probably made the most difference in the doctor I picked. 		<description>I had my lasik done here in Minnesota at LASIKPLUS. They have clinics all over the US. Go to their website and put in your zip to see where the closest one to you is. Cost depends on what type of surgery you would require. They do free consoltations and will check your eyes to see if your even a candidate for lasik, then if you are, they will go over what type and costs. Mine cost me $3500(both eyes) but by going through them and I keep all of my after surgery appts, I will have coverage for life so if anything goes wrong or I start having problems, I can go back in and get my eyes redone at no cost.
